What is ADAP?
ADAP is an acronym that stands for Alcohol and Drug Awareness Program.
It will be given in either your high school health class or you may take it online.
If you took health at Eastside and need your ADAP certificate, please complete the Google form below. Please allow at least 1 business day for a response.

If you did not take your ADAP test at Eastside, you will need to complete it online. Click the link below.

Certificate of School Enrollment
You might also need a Certificate of Enrollment from Eastside to say that you are a current student in good standing. Please stop by the guidance department at Eastside to request the form. Please allow a 24 hour notice as the form must be notarized.
